Medical Forms
A medical form needs to be filled out for each person attending the camp out (parents, scouts, grandparents, even cousins that might attend). A paper copy of your insurance card needs to be attached.
Note: Printed paper copies of the forms and insurance cards are required, electronic/digital forms are not accepted
